Children's E-book Permission Request & Licensing Form

For schools and non-profit organizations requesting permission to use our children's e-books. Complete this form and keep a copy for your records.

License Terms

Licensor: Barbara A Casper/ Follow the Fireflies, LLC

Licensee: The requesting organization named organization(“Licensee”)

Grant of permission. Upon receipt of the Permission Request & Licensing Form, Licensor grants Licensee a non-exclusive, non-transferable, revocable permission to reproduce and share the listed publication(s) only for educational or charitable purposes consistent with Licensee’s mission.

Allowed uses. Licensee may: (a) print and distribute copies to students/participants/staff; (b) share digital copies internally (e.g., password-protected systems); (c) display reasonable excerpts in slides or handouts; and (d) post the material publicly only if the “Public Posting” box below is selected and the attribution is included.

Restrictions. Licensee may not: (a) sell, rent, sublicense, or otherwise commercially exploit the publication(s); (b) remove copyright notices or credits; (c) claim authorship; or (d) create derivative works that materially alter the meaning without written permission.

Attribution required. Licensee must include the following credit line on every copy/posting:
“Used with permission from Follow the Fireflies, LLC. © [Year] by Barbara A. Casper FollowtheFireflies.com.”

Term. This permission lasts for one year if unspecified, unless revoked. Licensor may revoke permission for breach of these terms; Licensee must then stop use and remove public postings within 10 days.

Ownership. Licensor retains all rights not expressly granted.

No warranties. Materials are provided “as is.” Licensee is responsible for how they are used in programs.
